Liability Waiver

*To be signed in person before your first class.

I hereby acknowledge and agree to the following:

• I attest that I have no medical, psychological or emotional condition that would prevent me from safe participation in any yoga class offered by Pamela Ferman.

• I understand that Yoga classes with Pamela Ferman are not a substitute for medical attention, examination, diagnosis or treatment. Even with clear instruction there is the possibility of injury (e.g muscle strains, loss of balance, falls) and that it is my responsibility to consult a physician regarding any and all matters involving my personal health and including with out limitation:

  • Any prior or existing injuries, medical conditions and/or disabilities; AND

  • my ability to participate in the programs offered, before coming to a class run by Pamela Ferman.

  I release and discharge Pamela Ferman from all liability, claims, demands or actions that I may take resulting from injury, death, or damages arising from my participation in the physical activity classes offered by Pamela Ferman.

• I understand that Pamela Ferman is not responsible for the loss, theft, damage of my personal items.

• I understand Pamela Ferman reserves the right to refuse service.

• I FREELY ACCEPT AND ASSUME ALL SUCH RISKS AND THE POSSIBILITY OF PERSONAL INJURY, DEATH, OR PROPERTY DAMAGE OR LOSS RESULTING THEREFROM.

• I agree that I am above the legal age of majority in my province or territory of residence (19 or older in BC) and do not require parent or guardian consent.

  I have read this agreement & fully understand its contents & meaning, and sign it of my own free will.
